Venu K. Thirukonda, MD, FACP treats all forms of cancer and blood disorders at Northwest Georgia Oncology Centers, P.C.

Dr. Thirukonda received his Bachelor of Medicine & Bachelor of Surgery (MBBS) Madurai Medical College, The Tamil Nadu Dr. MGR Medical University in India, and conducted his residency at Maimonides Medical Center Brooklyn, NY and fellowship at the Albert Einstein College of Medicine Montefiore Medical Center Bronx, NY. He is currently active in cancer research and clinical trials to provide his patients with the most advanced cancer treatment available in Georgia.

Dr. Thirukonda provides patient-centered cancer care at NGOC’s West Georgia Medical Center.

Form 1095-C

Due to new federal legislation designed to reduce paperwork, the 1095-C will no longer be automatically mailed to you.  Form 1095-C provides information about the health insurance coverage you were offered during the year if you are or were a full-time employee. You do not need this form to file your income tax return; but a copy is available to you.

Contact HR to receive a copy of your 1095-C. Forms will be provided within 30 days of your request.
