Sujatha Hariharan, MD grew up in Houston, TX, and pursued her love of medicine by training at prestigious institutions around the world.

Upon completion of her Fellowship in June 2006, Dr. Hariharan joined the cancer care team of Northwest Georgia Oncology Centers, P.C. (NGOC).  With a special interest in treating gastrointestinal cancers, Dr. Hariharan is actively involved in cancer research and clinical trials to find a cancer cure and provide her patients with the most advanced cancer treatment available in Georgia.

“The treatment for colorectal cancer has changed considerably in the last 10 years due to translational research and patient participation.  I believe patients derive great satisfaction from being involved in clinical trials since they are not only helping themselves but patients fighting colorectal cancer in the years to come.”

Form 1095-C

Due to new federal legislation designed to reduce paperwork, the 1095-C will no longer be automatically mailed to you.  Form 1095-C provides information about the health insurance coverage you were offered during the year if you are or were a full-time employee. You do not need this form to file your income tax return; but a copy is available to you.

Contact HR to receive a copy of your 1095-C. Forms will be provided within 30 days of your request.
