Bradley J.G. Larson, MD joined the cancer care team of Northwest Georgia Oncology Centers, P.C. (NGOC) upon completion of his Fellowship at the University of Florida in 2004.

Dr. Larson is actively involved in cancer research and clinical trials to find a cancer cure and provide his patients with the most advanced cancer treatment available in Georgia.

Dr. Larson provides patient-centered cancer care at NGOC’s Carrollton Cancer Center and Bremen Cancer Center.

Form 1095-C

Due to new federal legislation designed to reduce paperwork, the 1095-C will no longer be automatically mailed to you.  Form 1095-C provides information about the health insurance coverage you were offered during the year if you are or were a full-time employee. You do not need this form to file your income tax return; but a copy is available to you.

Contact HR to receive a copy of your 1095-C. Forms will be provided within 30 days of your request.
