David D. Shepard, MD

Dr. Shepard, MD has joined the cancer team of Northwest Georgia Oncology Centers, P.C. (NGOC) He earned his Medical Degree from the Indiana University, School of Medicine in Indianapolis, Indiana. Dr. Shepard completed his internal medicine residency at St. Vincent Hospital, in Indiana. His fellowship in hematology / oncology was completed at the Comprehensive Cancer Center of the University of Michigan.  He is board certified in medical oncology and hematology. While actively providing patient care, he was an appointee to the Emory Breast Program Leadership Committee and the American Society of Hematology Trainee Council.

Dr. Shepard is recognized as one of the Top Doctors for excellence in Hematology and Oncology, in the Atlanta Magazine in 2020,2021,2022 and has been named a Castle Connolly Top Doctor of Atlanta for 2023. Dr. Shepard provides patient-centered care at NGOC’s Vining’s Cancer Center.

Form 1095-C

Due to new federal legislation designed to reduce paperwork, the 1095-C will no longer be automatically mailed to you.  Form 1095-C provides information about the health insurance coverage you were offered during the year if you are or were a full-time employee. You do not need this form to file your income tax return; but a copy is available to you.

Contact HR to receive a copy of your 1095-C. Forms will be provided within 30 days of your request.
