Rohini K. Chintalapally, MD treats all forms of cancer and blood disorders at Northwest Georgia Oncology Centers, P.C. She has a special interest in treating breast and lung cancers.

Dr. Chintalapally received her medical degree at Kasturba Medical College in India and conducted her internship, residency, and fellowship at the Medical College of Georgia in Augusta. She is currently active in cancer research and clinical trials to provide her patients with the most advanced cancer treatment available in Georgia.

Dr. Chintalapally provides patient-centered cancer care at NGOC’s Douglasville Cancer Center.

Transparency in Coverage Rule

Form 1095-C

Due to new federal legislation designed to reduce paperwork, the 1095-C will no longer be automatically mailed to you.  Form 1095-C provides information about the health insurance coverage you were offered during the year if you are or were a full-time employee. You do not need this form to file your income tax return; but a copy is available to you.

Contact HR to receive a copy of your 1095-C. Forms will be provided within 30 days of your request.
